Terms & Conditions of the <Client> cashback

  1. The promotion applies only to purchases of <product list> between <date1> and <date2>.
  2. Your cashback claim form must be submitted electronically, and printed and posted, together with the product barcode and copy of your receipt, to <address> before <date3>
  3. Ineligible for these cashbacks are <list of ineligibles>.
  4. The promoter accepts no responsibility for claims lost in the post.
  5. You should expect to receive your Universal Gift Card about 1 month after submitting the claim form, but it may take longer – please contact us if you have not received it within 2 months.
  6. This offer cannot be combined with any other offer.
  7. The promoter’s decision on all matters relating to this cashback promotion (including eligibility for the cashback and verification of claims) is final, and no correspondence will be entered into.
  8. Your submission of the cashback claim form will be deemed acceptance of these terms & conditions.
